If you are attending Harmony University and wish to use Missouri Western's workout and Health Center facilities, you must download and send the forms below to the address indicated below:

Pool Facilities in Looney Complex
If you wish to use the pool facilities in the Looney Complex, there is no need to fill out the forms listed above.
